My silkie's vent won't heal, pleae help :)

Hello everyone! This is my first post, hopefully I get all the info yall need. I started chicken keeping almost 2 years ago now, but I'm no expert!!
Poppy is a 7 month old silkie hen. She is smaller than her sister's, I can tell she is underweight. I noticed there was a problem with her a couple of months ago. I was putting my flock away for the night and she was the only one who hadn't put herself away. I found her out in the yard spinning circles! I thought she had been spooked but she was actually picking at her vent. I brought her inside and found that her vent was pretty much crusted shut. I researched vent gleet and started her on miconazole 7 and epsom salt baths every day. That didn't work so after about 3 weeks we switched to an athletes foot cream and continued with the baths. After about 2 months of doing this I am sure I'm missing something. I stopped the creams and switched to coconut oil and the epsom salt baths. Her vent has improved but it looks like her vent is still partly blocked. She is the only bird out of my whole flock with the problem. She eats and drinks very well. When she trys to poo she strains and clucks like it is difficult to pass. Her turds are only pencil width, very small. She picks at her rear end all the time, I'm not sure if she is trying to help herself poo or if its itchy. I would like to be able to solve this at home if possible, but I will take her to the vet if it is the only option. I keep her in the laundry room, she has a small coop under the table, I've brought her sister in to keep her company, and they have a kikiriki rooster with them.
