Need Advice PLEASE!!

I have a sick hen & it is 2 weeks now that she has been sepatated from the flock. I noticed her barely moving. I treated her for worms, not much of a change. I started her on kefir & she pasted a large rubber mass. Then she appeared to be on the mend until her wattle started to droop and become pale. The entire duration she has had white foamy diarrhea or it has a green matter in it.I have read so much & just don't know what to do next. She'll eat a few meal worms some scramble eggs and a little bit of mashed pellets. The diarrhea hasn't changed & it is over 2weeks. Should I worm her again? Can you do it that close? Should I put het back on the kefir milk? Also I wasnt sure if she should have plain water or apple cider water.How long can she go on like this? Help!!!
