Need Advice

Hi. I am a new chicken mom. I got 4 chicks, a Buff, a Brahma, and two Americanas. They are about 20 weeks old now. I just lost my second Americana this morning. The first one passed about 3 weeks ago. Both exhibited the same symptoms and I can't figure out what went wrong and if it will happen to my last two. Both of them seemed a bit quiet, but were eating and drinking... by morning, they were gone. What happened?
I give them corn mash, they get electrolytes and apple cider vinegar on alternate days. Their coop is very clean. The get clean water and food everyday. I feed them vegetables and fruits once in awhile and they are out of their coop at least a few hours every day.
They also have a dust bath with soil, sand and diatomaceous earth to keep away the mites and fleas.
WTHeck is going on? Am I doing something wrong, or are chickens really this delicate?
 
Corn mash is low in protein as are veggies.

Your birds should be on a formulated feed that has at least 16% protein, higher if you give veggies or scratch.

Can you feel their breastbone (keel bone) when you pick them up? If it is prominent they are underweight.
I fear they are starving with corn mash as the primary feed.

Have there been any other symptoms?
Diarrhea, swollen or runny eyes, standing or walking "funny"?
Anything else you can think of?
Keep an on the others for symptoms you may not have noticed with the others.
