What entry form do you need to fill out? Perhaps posting a photo or link would help us know what you're dealing with. Entry forms vary depending on the show, and I can't really help unless I know exactly what form you're using.
As for what to do when the judge comes around to your chicken, are you showing at a small show/county fair, or a larger open poultry show? At many (not all) county fairs, you have to bring your chicken up to a row of cages for judging. Put the bird in a cage and stand behind the cage. The judge will examine your bird, compare it to other birds in the class, place it, and move on. If the judge needs you to do something, he should ask.
At other shows, you leave your bird in a cage where it is staying. The cages are usually grouped according to breed, variety, and class. At shows like those, you don't have to do anything at all during judging. You can stand nearby and watch the judge place the birds, or leave and come back later to see how your bird did.
Regarding identification, that varies, too. If your entry form requires a bird identification number, then yes, you will need to get a numbered leg band for your hen's leg. If the entry form doesn't require a bird identification number, then you don't have to get a leg band. Leg bands can be helpful, though, even if they aren't required, since it helps prevent your bird from getting mixed up with other similar looking birds.
