His mouth is open and he is panting. He has been doing this for weeks, but my mom keeps brushing it off as something else. He is also stretching his neck out alot whenever he does this (which is very often)

I know what chickens yawning looks like. That's not yawning.Since he has been gaping for weeks, it's not gapeworm. If it were, he would be dead by now.
However, capillary worms in the crop could be a possibility. Only a fresh fecal sample taken to a vet will tell.
Other possibilities are; he yawning alot. Chickens yawn like people, like dogs. More frequent yawning and neck stretching can simply be crop or gizzard adjustments.
Swallowing a foreign object and getting it stuck in the esophagus is possible.
Finally, there's the possibility of a respiratory disease, mucus buildup in the windpipe and he's tiring swallow or expel it.
Otherwise, has he been eating and drinking normally?
My chickens stay inside most of the time, either roaming around the house or in their cage. The only symptoms he is showing are the neck stretching and panting. Oh, and he sneezes occasionally when I sweep, but thats probably from dust. His crop feels flat. But that's probably because he was taking a nap and he hasn't eaten in a little while.Check his crop to feel if it is full, empty, puffy or dough-like. Crop problems can cause gaping. More than like it could be a respiratory infection. Does he have any sneeze or cough, gasping, wheezing, bubbly eyes, or nasal drainage? Is it cold where your liver now? Do you use any heat in your coop? How is the overhead air circulation? Do you have any mold or wet conditions?

I'm in agreement with Eggcessive and suspect the onset of a respiratory disease due to the sneezing, gaping and neck stretching to loosen mucus in trachea in order to breathe.
Besides, gapeworms are rare in chickens.
