I need some help as to what could be wrong or what other steps to take.

Inspection yesterday was all fine. Today, one juvenile looks lethargic/tired, moving slowly, puffed up, hardly eating, and shaking his head regularly.

Currently in hospital set up with mash and vitamins added to water. Observed drinking, has not eaten.

Housing is a small wooden coop with run access, roost bar and dust extracted shavings. Run substrate is dirt.

Crop completely empty, colour seems good, mouth/nostrils clear, breathing sounds clear, eyes clear, thoroughly inspected for lice/mite and all clear. Only pooped once since moving to hospital box, and that also looked normal. Will attempt to include a video.

Forgot to add, no new arrivals and and was hatched by myself.

Other birds currently all look happy and healthy

Likely Coccidiosis. Give a drench dose for three days in addition to the Corid solution, instructions below. The drench dose is .5ml undiluted Corid directly into the beak once a day for three days. Only give feed with Corid, no vitamins. They respond fairly quickly to this, so then you should treat the whole flock with the Corid water. https://www.backyardchickens.com/threads/corid-oral-drench-instructions.1211991/

I just wanted to give a quick update, this has worked brilliantly and he's looking WAY better, I can't thank you enough.
