Record keeping chart?

Hi! I'm now up to 6 peas! I've been looking for a chart of sorts to keep everything straight. (hatch date, deworming, all in one, and anything extra) Does anyone have something like this that I could copy off of? they are all 2017 hatches but eventually I'm hoping to keep the breeding records and colors straight as well. I make my charts in Excell, you can put anything you want to keep track of and even have it on your phone so you can have it handy when you are ut with the birds. Before that I just kept notes on a calendar.
