I lost a hen to the same thing about 2 months ago. got real lethargic, butt tucked in , would go out with others but then would just sit there like she lost her best friend. saw that she had her rear all caked up so I gave her a nice warm bath and cleaned her up. knew she felt bad as I have never picked her up without a fuss. so yesterday she went into pen by herself and I gave her some dried meal worms. when she put her head down to pick them up a lot of brownish liquid came out of her beak. did that three different times. her poop is runny also. any ideas? I don't know squat about chicken illnesses. last one died quickly.
 
This is how some of mine have seemed when dying. A necropsy by the state vet or poultry lab is the best way to find a cause of death. What I do when one seems sick, is to check the crop for problems emptying, look for weight loss in the breast muscles, check for egg binding, look for lice and mites, and note if they have been recently laying eggs or have had problems laying. With a sick chicken, I will offer some chopped egg, wet chicken feed, and mix a tsp of yogurt into feed for probiotics daily.
