Very thin hen, abnormal poop

One of my favorite hens has been acting a little less active lately, but I didn’t think much of it until last night. I noticed that she has goopy cream-colored poop, and it gets stuck in her bum feathers a lot. Her comb is also pale, and she walks with her head tucked in. This morning when I picked her up I realized she was super light and extremely thin. I’m not exactly sure what’s wrong, but I really don’t want to lose her so if anyone knows what’s going on or has any advice I’d really appreciate it!
 
that probably means she is either not getting enough food or water or both so I would try giving her more food and water and if that does not work I would try calling the vet near you and seeing if you can take her in and you might check for chicken mites they will be like red white and maybe clear but they are also very small they are maybe the size of a seed tick and if that is the case then you are going to have to give all of them a deep cleaning with the following to clean the mites off of them and then you will have to wash the towels seperate from your regular towel laundry and then you will have to take a really good shower because the mites will end up crawling on you I will add a picture of chicken mites for you so you know what they will look like but you will have to move the feathers a little bit in order to find them but here are the following ingredients to bathe your chicken in if it has mites and the picture of the mites:
