Weak chicken mystery illness

Hi, I'm hoping someone might be able to advise me on a problematic Polish hen.

She's always had a fluctuating weight, from skinny to near normal but always bounced back. She's only ever laid twice and that was six months ago and never laid since.

She's now gotten very skinny in the past few days, and is unable to walk around much as normal because she's weak.

I've wormed them two weeks ago and she has had a partial moult. No physical injuries and doesn't appear to be distressed. She is eating. I put down porridge with vitamins mixed in to help boost her and she is eating some of the normal layers pellets. I give her some meal worms and mixed seeds as treats. I have also put crush raw garlic in their water.

Is there anyone that may have a idea as to what's going on. My other chickens are all fine. I'm in the UK. Thanks.

Does she have mites, canker, any respiratory abnormalitys, how old is she, how cold is it where you are, and can you post a picture?

Try to cut her crest feathers so her eyes are clear from obstructions. I lost a polish because she went blind, starved, and I didn't realize until she was already paralized from vitamin deficiency.
