When do I need to sanitize incubating eggs

You should not wash or sanitize eggs you are going to incubate. This washes away the protective bloom from the shell which opens it up for bacteria. Then in return will cause the developing chicks to get bacteria and not make it through incubation.
 
Should I only sanitize the eggs when dirty or can I still sanitize the eggs even if there is no visible dirt. Or do I need to sanitize the eggs even though there is little to no dirt. I use hydrogen peroxide.
Do not clean or sanitize your eggs you plan to hatch. It wipes the important "bloom" off the egg and the chick will not develop properly and will die before hatch date. It's fine if the egg is dirty, the dirt normally comes off throughout incubation.

I've never bothered, personally.
Some people do, I dont think its necessary or particularly wise.

If the eggs are dirty, I clean the eggs with a 50/50 solution of peroxide and warm water and cotton balls, right before I put them into the incubator.
It's not true that cleaned eggs won't hatch. I have an amazing hatch rate on all my fertile eggs.
Do not submerge the eggs though, just wipe them.
