What a bunch of work you just did for the benefit of your chickens, and ours. Well done! For those who want to geek out on maximizing their nutrition, you've offered some really nice data here.

I will say that what this shows me is that despite the notion that somebody knows exactly what chickens need, it is very likely that nobody really does. Just like the people RDAs we are guessing. Nobody has the kind of money to invest in chicken research that it would actually take to know this for sure.

Very comprehensive and worked out in easy to access ways. I prefer the OpenOffice (now LibreOffice) version. A must red for every chicken owner.

This is great information. Could this not be packed in a zip file and uploaded to a file hosting site?
Is the author interested in expanding this for vitamins outside the USA?

It is in zip format hosted on this site. Look for the section that says attached files. It's over on the right side. What vitamins would you like added?

So it is.
I use a couple of vitamin supplements that are available here. I'll post the details in a comment with this article when I get a chance. :)

If you use commercial feed, and offer table scraps/veggies on the side, do you still have to give them vitamins?

A good quality feed should contain all that your chickens need. Adding table scraps and treats in the mix messes with that formulation and that is why you are not supposed to give more than 10% treats. It could get very complicated trying to give scraps as a main source of feed then trying to supplement with vitamins and minerals for what was missing.

If you are using a good quality commercial feed, you should not be feeding table scraps as this dilutes the nutrients in that feed. I consider this a waste of money! Not to mention the damage you are doing to your chicks.
