This would be the hen, I know I had a hen who was notorious for eating and pecking a "small hole" in all of her eggs.
I would look over their diet and make sure that they are getting everything they need. Try your best to stop it, you don't want it to become a habit with them

I found a way to help my chickens when I had them to stop picking on each other, I would supplement their diet with crickets, and worms. Mostly crickets though, It gives them something to do, and is a pleasure to watch the chickens run around and enjoy chasing them.

As far as the chicken, I have had signs of rats attacking my birds before. Never killed one though...
