Have you considered changing the RH during the incubation period to the dry method. Maybe the 40-45% is a bit high. Many people find they have a better hatch with this methods. Since you have tried the higher %RH 2 times; perhaps a lower 30-35% is worth trying.
Do you track t he air cell development to verify the loss of moisture in the egg? THe chicks cannot pip and zip if the chicks are too swollen with fluids. THere is the weighing method and the candling method. Here is a good diagram used with the candling method.

Sorry this second batch didn't result in any chicks. Try to remember how many times Edison tried and failed to make the light bulb. You learn each time what doesn't work. YOu will eventually find the way that DOES work. GL
