Accidental crop injection?

All the girls in my flock are mycoplasma carriers (most likely) thanks to a careless rescue last year. Everybody's usually fine, but 3 have a bit of a cold at the moment. They've been tolerating their Tylan shots beautifully and seem to be better. While giving them their last round yesterday, one of them kinda flipped out and I ended up sticking her in the crop. At least I think that's what must've happened. She went from angry-but-fine ... to blue-combed, gaspy, vomiting and dead within about 5 minutes. The only thing I can think of is that I maybe punctured her crop? Can't find any answers and want to make sure that's what happened and I don't have some sudden-death illness that's going to affect the rest of them! I feel awful!
 
Injecting her crop would not have killed your chicken. She may have just been so stressed out or weakened by her respiratory disease that she had a heart attack. Tylan is frequently given orally instead of by injecting it, and that might be something you might want to do instead. Sorry for your loss.
