Have you fully excavated the unhatched pips that died? Sometimes, they can be in a malposition which makes it impossible for them to hatch by themselves - typically, they can't zip at all if this happens.
I've only ever had one and he was head over wing and made it out fine with assistance, but was lame for a few days after.
Could also have ruptured an unabsorbed blood vessel and bled out.
Opening them up and having a look will be best.
