Need help with foot
Only the top
Had previously soaked in Epsom removed most of scab. I also treated with anti fungal. Grew back
Hen does limp and stands mostly on 1 foot

How cold is it? Could she have gotten it wet in freezing temperatures? It could possibly be frostbite if it is winter weather.
not cold and had this in warmer weather. soaked in Epsom salt most actually peeled off leaving fresh skin in leg underneath, Then grew back. was treating with antifungal and that did'nt work
 
I'm in agreement with @GodLovesU that this is scaly leg mites. I see no swelling above the blackened tissue that would indicate the black is necrotic. It appears to me that the black is mite offal, signalling a serious leg mite infestation.

I suggest soaks in liquid permethrin or spinosad (Elector PSP), both safe insecticides for chickens.
