Blind sunken eyed chicken

We have this 2 year old hen and her eyes have started sinking and she needs help getting back into the coop at night as I don’t think she can see. Should we take her to a vet and what is going on with her?

What does the other side of her face look like?
She has some swelling around her face, eye and ear - hard to know what's going on there. Is that discharge or dirt in the corner of her eye?
Look inside her beak - is there any yellow/white pus, canker or cheesy-like material in there?
Any coughing, sneezing, rales/wheezing?
Look her over well for lice/mites while you are examining her too. Check her head really well, looking in and behind the ear too, just to make sure there's no bugs that could be causing the irritation.

If you have vet care, that would be best.

General care for the eye - I would flush with saline, remove any debris/pus, then apply an eye ointment like Terramycin or you can use plain Neosporin.
