Chick acting lethargic, limping and not eating.

So I have my girl, Janeway. I have had her for about 2 months. She is an olive ether and is around 12/13 weeks old. Lately she has been walking as if she is too heaven for her legs and losing her balance. She eats and drinks but not a lot. She has lost weight. The vet has prescribe a dewormer for her. She has been on this for about five days. But has not really improved. I have been giving her boiled eggs and a few treats but not much so she still eats her medicated food. Her poop looks normal. I have also been adding poultry tonic to her water to giver vitamins.
Though she has been on the dewormer for five days she has not improved much. She is still alert but would rather sit on her perch or in the corner. She still walk like she is to weak. Any suggestions on what might be wrong or how to help her.

She is in the house and in her own box to isolate her.

While on the Flubenvet, she should probably just eat that so that she gets the correct dosage of medication. Can you get a picture of her legs from the front to see if there is a bone deformity? Does one leg seem weaker than the other? Is Mareks common in your area? If you can get some riboflavin (vitamin B 2) or B complex, I would give some daily in her food or orally. Look her skin over for lice and mites under her vent, neck, belly and wings. Mites can make them weak.
