I think from what your saying the chick has pipped at the pointy end of the egg and not the fat end where the air sac is? If this is the case then yes it has pipped wrong and is a malpositioned chick. If it has made a pip hole there is nothing you need to do for at 24 hours yet. The pip hole it has made will give it a good oxygen supply for now. It will still need to absorb yolk and blood vessels at this point as this external pip is its internal and external pip all at once. Time is the key to malpositioned chicks so in the mean time give this link a good read it has a whole section on assisting malpositioned chicks

Try not to open your incubator you need to keep the humidity up as there is a risk of shrink wrapping which will add to its problems. Some malpositioned hatch ok some need assistance but there is always a chance that they won't make it.
