Chicken had head resting on ground and is wobbly

Hello all, when I went out to clean the coop this morning and let my girls out to free range I noticed one of them was just standing in the run with her head resting on the ground. I rushed to get her and check her out and she was very wobbly. She wasn't moving around much, but she was eating and drinking a little. She started walking around a bit, but stopped and rested her head on the ground again. I separated her from the rest of my girls, but I am so worried about her. Any help or information would be greatly appreciated! Thank you so much!
