Chicken Losing Weight

I have a 2 year-old-buff orpington-hen who is puzzling me. She used to be 7-8 pounds....she is now lighter then my leghorns. She is eating/pooping/drinking fine. Her feathers are soft and shiny, though they are a little ratty per a picking problem. The only really "off" think I noticed besides losing weight, she once in a while looses her balance and has to stick out a wing to right herself. She goes out and forages with everybody, and runs for treats when I give them any. It is very hard to tell if she is sick or not - she has always been slow/sluggish and I think may be partially blind. Anyway, I was wondering what might be causing her to lose weight? I wormed my chickens awhile ago with ivermectin, it was probably over 6 months ago. They also I THINK had cocci about 6 months ago...gave them some medicine and they are fine now. No other chicken is losing weight/acting sick. They all have shiny soft feathers, though many of them have missing feathers because, again, a picking problem. She is they only one to be having 'problems'. I did have a chicken die in Feb. 12th from either sour crop or egg yolk peritonitis, not sure which.
