chicken with loose stools and vomit???

This is not the first time I have written about this, it seems like every couple weeks this is happening again. My Chicken is sick , mostly lethargic and then when she moves she vomits watery vomit, a lot! I don't even want to pick her up its so disgusting. Why am I losing my chicks. Last time we medicated their water but it didn't help the sick chick and we had to put her down. They are about 2 years old or more. Any idea what is happening? Help!!
