Chickens dying randomly

Does this happen to any one else? I have had two chickens die on me this summer. One day they seem fine the next we find them in the run just flopped over! What the heck?! I know chickens hide disases like no tomorrow. I'm worried about my other girls. We did have our rooster die on us like this last year and no one else died. I get so upset when my girls just die ;(
 
The latest one was a gold sex link, she was about 3 years old. The other was a EE and about 2. Those were my pure breeds. Most of my others are "barn yard specials". As I have a rooster and let hens go broody. I have switched out roosters 3 in 3 years.

They have a diet of tractor supply chicken feed, oysters shells, scratch and , veggies, bread, grass clippings and such when we have the left over for treats. Oh and occasionally meal worms.
 
Stop feeding them bread, that can cause impaction and death- not saying that is what happened here, but bread is dangerous.

What kind of chicken feed? Layer, grower?

What have the temps been like where you live? It may have been heat stroke.
