Dead hen with about two inches if guts hanging out of rearend??

We are new to raising chickens and don't know too much about it, so we are turning to you guys. This morning I found a dead hen with about an inch to two inches of her insides on the outside? Any help would be great.

It could be a prolapse. How old is the hen?
Were her insides coming out of her vent? (Area where the eggs come out?)

She was about a year old and they where coming out the cloaca.

I'm thinking she prolapsed. I've seen this more in older hens that are finishing up their lay cycles but I have also seen it in young ones. In a young hen, I'd tend to think that it may have had to do with her reproductive anatomy on the inside. Some times it can be genetic or just an anomaly.

I forgot to mention that she had a lot of poop on the feathers around that area (diarrhea)?
