I have had a few times where antibiotics were needed and finding the doses online was very difficult.
Here it is so I can find them easier in the future:

Oxytetracycline HCL Powder:
Used 800mg dose= 2.5 Tbsp per gallon of water for 5 days
Soluble Powder A broad spectrum antibiotic. For control and treatment ofspecific diseases in poultry, cattle, sheep and swine.
One packet contains 10grams of oxytetracyclineHCl.
181.5 g (6.4 oz) pouch,
25 pouches per case
UPC# 7-45801-10571-3
To make 1 gallon:
400mg dose= 1.25 Tbsp per gallon of water
800mg dose= 2.5 Tbsp per gallon of water

Metronidazole Fluid: I don't have mg/ml conc. or sources
Used 1000 mg/L in drinking water for 5 (to 7) days
50mg for a pigeon for 3 days and 60 mg/kg for a chicken for 5 days
For 2kg bird give 1/4 of a 400mg flagyl daily

SMZ/TMP tablets:
Gave 1/4 tab 2x daily for 5 days to the 5lb pullets
'Just to let you know, here's the dose if you had a parrot (not that you would necessarily want to give this to a chicken): for a 4 pound bird give 192 mg (a fifth of an 800mg/160mg combo tablet) once or twice a day for 5 days' from: http://www.justanswer.com/veterinary/3nkhk-use-sulfamethoxazole-trimethoprim-chickens.html
