Eggs develope fully grown chicks, but then they don't hatch and die!

Please help me. I am getting so frustrated with this hatching process which sounds so simple when some of you guys describe it.
I have one Brinsea Mini Advanced, one Brinsea Octago (fully automatic with Humidity sensor and regulator) both with automatic turners.
All my the eggs that show initial embryos on day 4-5 when candling them end up developing fully grown chicks, but then 2-3 of them out of 10 hatch and the others die. I did test the accuracy of the thrmometer and humidifier and everything seems to be very accurate. I set the humidity usually at 50-55% initially and then increase it to 60-65% for hatching. The temperature is usually set at 99.6.
What am I doing wrong?
Any suggestions are appreciated.

I don't know the cause of that - I had one do it on my last hatch too, though. Its really crummy

I did the dry hatch method and I ended up with a hatch rate of 75% on my first try. And using an unpredictable LG styrofoam 'bator. You might try that??? It seemed to work great.

Are your eggs shipped or yours/local? I have the Brinsea Octo advance too and have had good luck with it. The only difference that I see in settings is that I keep the humidity at 45% the first 18 days and then up it to 65% for the last three. I also keep the vent hole open because I did read that the egg itself needs some oxygen. I have also read that too high of humidity keeps the egg from losing moisture and the chick may "drown" in the egg when it gets close to hatch. If you are doing shipped eggs they can often be a gamble due to the post office handling. I only have a few hatches so I am not anywhere near an expert, but those are the things I would think of first. I hope it gets better for you!

I had several do that on my latest hatch and dont know why, but allot of them were shipped eggs. I am wondering if it has something to do with egg care prior to incubating. I know that it has been so easy that I have been a little careless lately.
 
My hatch that just finished I got 5 chicks out of 7. 6 never did pip just died in the shell and 7 was facing the small end of the egg and died there.

I did the dry incubation btw with the LG still air
 
I just lost 16 eggs. They were fully grown chicks, just died. I wonder if it had anything to do with not having them in a carton. i was using a still ait lg. Im going to put a fan in and try again.
