Eggtopsy. Any thoughts on what happened?

Had this guy "pip" last night but it looked gooey. Nothing changed overnight so I took it out to candle. Looked odd and no movement so I cracked it open partially. Still a little squeamish so this is as far as I got.

Turns out the beak wasn't even close to the "pip" hole so I'm not honestly sure how the hole came to be. Too much pressure from the decay?
I'd really love to know what you all think and if there is something I could do to prevent something like this happening again.

I had pretty consistent humidity (around 48%) and most of the other eggs are hatching out fine (though two decided to pip the wrong end. I assisted both. I saved one and was too late for the other).

Power went out twice for about an hour each time, but temp never got below 95F

Most at hatch deaths or dead in shell quitters (I had one a week ago that zipped and then died!) are not a ‘if I had only X or hadn’t done Y’ scenario, there is just something developmentally wrong with the chick and it just isn’t going to make, even with the best off intentions! Sometimes you can see an obvious problem on necropsy but at other times you can’t. As long as it is 10% or less on a home raised, no complications hatch, it is just normal death loss. But with a stress hatch (power went out, shipped eggs, heat or cold exposure, 10+ days old at incubation) upwards of 50% death loss and deformities can be normal. On your particular egg it would be much more helpful to get it out of the shell for a proper diagnosis. The way the blood/fluid looks makes me think it died 12+ hours before you opened it. That dark red/purple, thick fluid, clumpy look on candling only happens after it has been dead for a bit, no idea where your pip hole came from unless it died shortly after pipping and then sat there for a day or more, decayed a bit, then rotated in the shell (the solid mass would float to the top)? Even when you send an aborted lamb/calf/dog into the lab you only get an answer 50% of the time so diagnosis isn’t guaranteed but you can certainly learn a few things by opening eggs!
