Fly strike? Worms?

My 2yo Buff died this morning. She had developed sour crop June 8. I had vomited her a few times and given her a week of monistat, and she seemed to be recovering nicely. She was just starting to eat again, have solid poops, and was moving around and acting like a chicken again. Then this morning, I found her on her side in the coop with a prolapsed oviduct covered in maggots and one other type of worm. Considering her already weakened condition and that she seemed to be pretty far gone, I euthanized her.

I gave all five girls safe-guard June 26, and am planning to give 2nd dose tomorrow. I'm wondering if the other worm I saw in Lily this morning was from the worming, or something else. It was like a horror film, and I didn't look too long. The other four hens seem to be doing fine, but I don't want to lose anyone else. Thoughts?
 
Yuck-o! But thank you. That's what I saw, plus a bigger grey worm. It almost looked like a tiny slug. Her butt had been poopy a few times after the sour crop, but I was keeping up with it. The other four are fluffy and clean. I think the prolapsed oviduct was the last straw. We have had so many flies out there the last few weeks; the fly strips I hang out are full and have to be changed almost every day. The cooler, drier air from Arthur has helped the last few days. We always clean the coop in the morning and keep sand in the run. Just really hoping no more birds are affected. Anything else I should be doing?

On the flies....I get these:



And boy-howdy do they work!!! No fuss no muss...when it's full I just throw it away. They're cheap and what a relief around the coop!!
 
Normally I would have looked at her myself, but the creepy crawlies were just too much.

I just looked at the NC Ag website, and I could have taken her to the closest lab for $15. Wish I had thought of it, but in my rush to clean up and get to work this morning, I did not. That would have taken away my doubts about what did or did not happen. I will know for next time. Thank you!
