I am planning to build a hanging heated nipple waterer. Planning to use a 2 or 3 gallon bucket with 4 or 5 nipples on the bottom and running a heating element through a hole in the top.

It seems like when I remove it to refill that the nipples would be on the ground, maybe damaging them and spilling water. Do I have the design right, what is the solution for refilling it outside the coop?

I use a bucket with one or two nipples in it. I just either hold it off the ground or rest it on a couple of pieces of 2 by 4.
It is not really a problem. I use those square kitty litter buckets.
