HELP Chick is bad!

Polish hen took a turn for the worst! I found her lying in the yard ina rain storm today!!! she is nothing but skin and bones I dont know how she got so thin so fast... I googled pedialyte and they say that will help since that is all i have on hand... but she seems some what alert but still not enough to do anything what to I do next? How much to I give her I am so sad for the little thing!!

Could have been a number of things. Worms, cocci, CRD, who knows. Ashland, Oregon? Looks like it's been a bit chilly (50s in the day?)... 5 weeks seems young, but maybe I'm wrong. Likely something that had been developing, or simply a failure to thrive. Are the rest of your birds alright?
