HELP Eggs Keep Breaking Under Broody- 1 Egg Left!

I have a broody Cochin name Luna, this is her and my first time.
Today I checked under Luna, I found an egg that was cracked open and the chick dead. Some shell was eaten. The first of the three eggs had broken on maybe day 2.

This chick was from egg 1, and yesterday I observed a very faint crack along the middle. Could she have stepped on it? I didn't observe blood on Luna's beak. Should I trust her with chicks and the last egg?

Recommendations?

(I removed dead chick and cleaned Luna up, I hope that was okay)
 
I have a Brahma and can be very clumsy. What I have learned is now I put a towel under her nest (this prevents her from making such a deep nest that the eggs are sitting on wood or plastic, depending on what she hatching in) I also give her a lot of pine shavings (12" or more) because she like to have a deep nest and unfortunately she still makes mistakes and has stepped on eggs and even chicks. The last two hatches she hatched in a large dog house and both time she stepped on and killed chicks, this last hatch being the worst she killed 5 of 9 that hatched

I was very sad. She is a very dedicated momma but do to her size she doesn't always see where her chicks are. This is her fourth hatch and she and I both have learned something new with each brood. I would trust your broody with the last egg. She is learning too considering it's her first hatch. When are the egg supposed to hatch? Are you sure the chick didn't start to hatch and just didn't make it?
 
My broody crushed an egg on day 19 and I found the dead chick. It looked like she had stepped on it because her foot had dried blood and cracked shell on it. She has 4 more under her and it's day 22 today. I'm still holding out hope.
I would trust your broody with that last egg.

The egg are on day 16, should hatch on Sunday, they are white leghorn bantam eggs.

One of the contributors on the Long and Informational Broody Thread keeps her broodies in a nest that is 2-4 inches deep of shavings (not cedar), topped by 2 inches of very high quality dried timothy grass (hay), the kind that is found in pet stores for feeding pet hamsters. The really good variety that doesn't have any stems, and is soft, really soft. I bought a small "bale" of American Pet Timothy Gold, which comes in a plastic bag about 1X1X2 ft, and cost's ~$7-8. But it was the best quality "hay" I've ever seen. Each small bale makes about 3 nests, and as skeptical as I was in the beginning, I'm now a fan. The nest is cushiony, yet held together, so the nests don't get so deep that the eggs are on the bottom and get crushed. That might help a little.
