I notice she been not here normal self and I picked her up and look what I found. I have no idea what to do.

WHOA, ok. Where is this on the bird? are there maggots in the wound? Start by trimming away all of the feathers surrounding the wound. Keep her inside and warm.
 
Does it smell bad? If so, it is infected. If it is infected In addition to cleaning and antibiotic ointment, I would recommend going to Tractor supply for a bottle of penicillin (G procain), this helps fight the infection. it would be in the refrigerated section and you would probably have to ask a clerk to get it for you. It is an intro muscular injection so you would also need to get a syringe and ask someone who works there what the smallest needles they have in stock are.

This looks like it's at the base of her neck, under her hackles. Is this correct?
Does it smell like anything?
Are there maggots?
Can you tell what exactly that brown stuff is?
How exactly has she been acting differently? Has she been eating and drinking?

Bathe and clean that. Can't really see anything past what looks like grime and maggots. Keep her warm and inside while you get her sorted out.

She's eating, drinking. Laying eggs. I believe my rooster talons ripped her open. It is under her wing.
