Help with dosage

I have a chicken who is 2.12 pounds and was on amoxicillin that was prescribed by a vet, due to a respiratory issue. After 2 week's he still has a snotty nose. Last night I noticed he has some bubbles in his eyes, meaning he needs Tylosin, not amoxicillin. I have an oral form called Amtyl, which is amoxicillin and tylosin, but the directions are confusing. We can up with the 2 pound chicken needing 9ml. See the attached directions and let me know if you agree. I want to make sure he gets enough, but not too much.

I read that where it says to give 1 ml of the mixture for 453 grams of weight per pound, and it seems like a lot to give. I might give it in the water at 1 gram per 240 ml (cup or 8ounces) of water. I am more familiar with Tylosin dosage when given in the water. That is given for 3-5 days. Here is where it is sold, and dosage is 1 tsp per gallon of water:
