Help with my Chicken!! is it sick???

I noticed today my chicken chicken is having trouble pooping i thought she a pastey butt. when i looked at it she has like a tumor or large lump coming out of her butt what do i do??????
 
It looks like a prolapse vent how do i fix it??????
