1) What type of bird , age and weight (does the chicken seem or feel lighter or thinner than the others.)
Baba, while leghorn, she's pretty skinny right now, probabl less than 4 lbs.
2) What is the behavior, exactly.
She's been acting lethargic, and throughout the weekend she went from lethargic, penguin pose and not eating, to moving around and eating on her own, to eating less and back to penguin position. She does try to walk around but it's clear she's in pain. I saw some yolk and eggwhite near the coop on Thursday evening, and she pooped a lot of yolk on Friday. this hen has never laid normal eggs, all of them have been somewhat deformed and huge, and she laid shelless eggs a couple of times before.
3) How long has the bird been exhibiting symptoms?

4) Are other birds exhibiting the same symptoms?
No
5) Is there any bleeding, injury, broken bones or other sign of trauma.
Nothing externally. she lost a lot of feathers due to our rooster despite the chicken saddle she had, and hasn't grown them back since last year.
6) What happened, if anything that you know of, that may have caused the situation.
I had made a new saddle for her to better cover her baldness, and I had added a strap on the bottom across her breast to better keep it in place. It was loose enough that i could get my fingers under it with no problems. she didn't like it and tried for 2-3 days to take it off. Then I found her hiding behind a board on Friday morning and I noticed she had become lethargic.
I thought it was due to the saddle that she was acting like that, but she didn't recover after I took it off. that's when I noticed she was pooping yolk and eggwhite. I thought that an egg had broken inside her, but I couldn't feel any eggshells inside her. I did some research and read about egg peritonitis, but her abdomen is not swollen. She started to make a weird squeaking/peeping noise when she poops, or tries to poop, since yesterday, and today her breathing sounds a bit congested, and when she tries to poop, I hear a sound that I'm not sure whether it's coming out of her butt, or her nose, but it sounds either like a fart or somebody blowing air through the nose hard. I haven't been fast enough to get close and listen when she starts doing it.
7) What has the bird been eating and drinking, if at all.
She ate a good amount of cottage cheese yesterday, and she ate some cucumber today.
8) How does the poop look? Normal? Bloody? Runny? etc.
Pictures at the bottom of the post, she pooped today again and one looked normal, albeit small, but now she's only pushing and there's a bit of whitish foam on her cloaca.
9) What has been the treatment you have administered so far?
I have her in my room in an open dog crate near a heater, and I've been giving her pedyalite as drinking water mixed with ornacycline. I couldn't manage to find a vet this weekend, and the oral baytril I bought hasn't arrived yet.
10 ) What is your intent as far as treatment? For example, do you want to treat completely yourself, or do you need help in stabilizing the bird til you can get to a vet?
Seeing how she's still weak (I was hoping she'd get better today), I don't think I'll take her to the vet anymore
. She looks so fragile that I fear that if she needs surgery, she might not make it. I guess i just want to do as much as I can on my own to give her a chance to recover, but I've only had chickens for 2 years, and this is my first time dealing with a problem like this.

12) Describe the housing/bedding in use
wooden pre-made coop, and I use pine shavings I bought from walmart. the chickens have full access to the fenced backyard, and we don't use chemicals to treat the plants.

The following video is from today, and Baba is more alert, even though she still gets fluffed up and looks still ill/in pain, but she's walking around more, and even ate some cucumber without any encouragement. the sound is rather bad, but you can hear her peep when she pushes to poop.
