Hen is thin with pale face

I have a Rhode Island Red hen who has become very thin over the past few days. Her comb has also become pale and dried out looking. Today I noticed that her legs and feed have become very yellow in color and the flock seems to be secluding her. She is eating and drinking fine and appears to be as active as always.

I have given her some wormer but I want to know if that's the right thing or if there's something else I should be treating her for.

I have attached photos below of her. The second photo is of my other hen for comparison. She used to look nearly identical to our other Rhode Island Red to the point it was difficult to tell them apart. Now it it's obvious who's who.

Worms could be a possibility, would explain the weight loss. Or, the other chickens aren’t letting her eat? You should have multiple feeding / watering stations. But if she has worms, they all have worms so I would treat the whole flock.
