Hen molting and bleeding

My flock has been going through a molt.. everything seems normal however 1 of my hens started pecking at herself this was the week of Christmas i removed her from the flock brought her in the house gave her a warm bath and kept her inside until she was all healed up she was one of the last to molt once she was healed and all feathers started coming in nice I put her back out with the flock, she was inside for al ittle over a week shes been back outside for about a week shew as fine when I went to feed them yesterday & check on them again last evening however when i went out a little bit ago she has her entire backside tore open 7 bleeding again.. I brought her so i can get her cleaned up & take care of her I just don't understand why she's dug herself open again.. Any suggestions???

She may not have done it this time. That's typically damage cone by other hens.

Normally I would agree but her mouth is covered in blood
