Hen throwing up with diarrhea

My favorite hen, Sesame is a 2 year old barred rock. A few days ago she got really sick. She's throwing up and she has diarrhea. Both are almost pure water with some green stuff in it. She is still walking around but she's more lethargic.. Going off by herself to lay down and rest. She was eating some but not alot until today when she was eating meal worms like she was starving.
I'm going to give her electrolytes but does anyone have any ideas?

I forgot to mention her feathers are falling out. She's been molting for awhile so I dunno if it's related but they are falling out again.
She does free range with her flock. No other chickens are sick.

We need more data. Can you feel her crop? Does her breath smell? What are you feeding her? Are you able to perform a fecal on her?
