Hen won't eat and is acting sluggish

I can't tell with all the fluff how big her comb is, but if she has one feel it and see if it is warm to the touch.
 
Aw, I’m glad you were able to catch her. I agree with another person, isolate her and feed her a scrambled egg/wet mash/canned tuna, and get some fluids in her. The electrolytes will help her too.
Plain geek yogurt is good for her too. How does her crop feel? Squishy? Hard?

It was a little difficult to find, it feels kind of squishy, but very small? I'm not sure how I should describe it. I just went out to check on her again and she's lying down next her food but she hasn't touched it yet.
 
These claws need a trimming and I would cut 1/3 of the total length. As she has light claws you will be able to see where the 'life' ends.

I would trim back some of her crestfeathers to allow for unobstructed view.

When did you last de-worm her?

I just realized, I don't think she's ever been wormed, or even vaccinated.
 
Wait, can you use diatomaceous earth for deworming? I thought I heard that somewhere and I have plenty of it from when my turkeys got mites.
No, DE does not work as de-wormer. You might want to use a proper de-wormer like Flubendazole 5% or similar.
