Hens and rooster attacking to kill a specific hen

I recently had a hen that the other hens and rooster were going after with the intention of killing her. I couldn't figure out why so I separated her and she was doing really well on her own but I found her a couple days ago dead in the coop. It looks like she went to sleep at night and passed away. Has anyone else had this happen?
 
She was most likely ill first before the bullying. If there's a sick bird in the flock, it's common for the others to attack. Think of it in terms of survival. A sick member will draw the attention of predators and slow down the flock. They also take valuable resources that would otherwise be used for the healthy members.

Harsh but survival of the fittest..
