Hey you Poultry Show savvy people, I need entry form help!

So I've only ever attended one fair before (did very well) and so now Alyssa (Silkieluvr) and I are taking the BIG step and will be attending two shows in January. Only one has the entry form up yet, and I'm afraid to say its not as easy to understand as the entry forms for the fairs. Here's the link to the entry form.

I understand checking the large or bantam columns, but I don't know what to put into class, or what the 'C H K P OT YT' stands for. Here's a run down of the list of chickens that may be going:

Barred Plymoth Rock Hen
White Bantam Silkie pullet
Mille fleur d'Uccle pullet
Mille fleur d'Uccle cockerel
Cochin pullet
White Crested Black Polish Hen

So does anyone know what I put into the Class section of that form?
Also, for example if I'm signing up my white bantam silkie, (I want to get this right, lol) do I put the 'bantam silkie' in the breed section and just 'white' in the variety, or do i put 'silkie' in the breed and 'white bantam' in the variety?

Variety is the color and class is like featherfooted bantam. you will be in the same class with say cochins if you are showing that breed. Orpingtons are in the English Class.Hope that helps. Some forms just ask for breed/color/cock/hen/pullet/cockeral . I'm sure they all have variations.
