how did this egg happen?

They may need oyster shell. Sometimes the girls just fubar.

Maybe cut out any treats for a while, sometimes chickens fill up on treats, and then they aren't eating enough of their regular food with nutrients and calcium.
 
Although I have no expertise on the subject, IMO it looks like it got twisted (before shell forming) and it continued to form until it was layed. It must of hurt coming out and I'm surprised it didn't become stuck or broken. I would think it was a one of a kind and would only be concerned if it happened again.
 
got one the other day almost like that only one tube and not 2 tho however wone could have been already gone. The shells are a great place to start however i got quite a few soft shell eggs in the days up to and soon after.From what i have leared and understand sometimes if a chicken becomes scared it can mess up the egg shell (soft, deformed, ect.) I have seen this a few times in the past. I was over run a for a time from halks diveing at chickens i lost 9 chickens in about 3 to 4 days. hope this helps some.
 
I wouldn't worry that they are eating more outside stuff and less layer pellet... that is good. I don't feed any kind of pellet, just let them range and give them grain in the afternoon when it is time to go back in the coop. Mine laid lovely big eggs right from the start.

If it is a one off no problems, otherwise I would do as suggested:
- no treat foods
- try increasing calcium and proteins


Very interesting egg though. I've never seen one like that.
