how do I write a proper 2 weeks notice letter?

Most of my jobs I have left with a verbal notice...

I drive a school bus for many years so when I left I simply checked the box that said I will not be returning in the fall....

when I left my job at the nursing home I simply did not apply to extend my contract....

others I have verbally given 2 weeks notice

I was transferred to the current position... but now it time to leave.

With the gas prices sitting at ~ $4.80 a gallon its no longer feasible to work in the community my fuel charge is exceeding my fuel allowance..I just paid my fuel card and my payment was half of my bi-weekly cheque

and now that gas spiked over the last few days (jumped more then $1.20) there is no way... that and the fact that the little 4cyl car I am driving will give hp the ghost soon which will put me in the position of HAVING to buy a newer car.. there is no way I can afford the car payments and the fuel... and still live..

so with my review coming up on March 7th I want to walk into my review totally prepared for everything... all my data... fuel costs, maintenance costs ect... I am hoping for a whopping raise but and not holding my breath so I also want to go in prepared to end the meeting with my 2 weeks notice on my bosses desk if I dont get some substantial raises (per hour raise and mileage raise)

I cant very well write "I quit" and sign it...

Would you address the letter to the boss/supervisor?.. the owner of the company?... or generic to whom it may concern?.... to management of (______________) company inc?

I would address it to your immediate boss and use some standard format. If your company has a standard that you can get from a coworker, I'd go for that. Otherwise, a good one to use is as a blank canvas would be the military 'Memorandum for Record' -professional and simple format. It's very blase and you'd have to take off the military emblems and change the header, but it's something to start with.
 
I would do a basic letter in writing without any reasons why. I think if asked you could verbally share that the cost of your job now exceeds the pay.There is no point in keeping a job when you are essentially PAYING to work. So sorry it has come to this for you.

Let's start from the beginning, it gets addressed to your manager/boss. Then the first line is about the letter, so since it's a letter of resignation, this is where you express the reason for the letter. You could write something frilly like "This letter is to formally express my resignation from such'n such position in the form of a 2 week notice, starting on --- and ending on ----.

You could continue with something like how you appreciated the position, but you don't have to give them like a top 10 reason list of why you're leaving. You can briefly touch on number one if you want.

All it really needs is dates and your signature for their records.

That's exactly what I would say/have said in the past.
