Pour about 2 inches of white vinegar into a plastic butter tub and let it sit on the dash of the truck for a day. Works good every time one of my kids puked in the car. Sometimes I had to do it two day in a row. Seems to work best on a hot sunny day.
 
I tried the Febreeze tonight hope it works. If not I will be trying the coffee and vinegar.

Might have found the main source of the stink though.
The floor mat. I washed the truck the other day and they have a floor mat cleaner. Took a sniff of the floor mat and boy did it stink. Threw it out.


I wasn't crying over spilled milk I was cussing a blue streak.
 
Still stinks this morning but not as bad.
Going to try the vinegar suppose to be a hot day.
If that does not work then the coffee and baking soda.
 
Get a bottle or two of 91% isopropyl rubbing alcohol and pour it right onto the carpet. Let it dry and air out and the smell will be gone. Be careful about getting it on other interior parts such as plastics or leather as it may discolor them. Repeat as needed. Sounds like you pushed the milk further into the carpet and padding when you attempted to clean it.

This is what I did after almost a gallon of milk leaked into my Excursion. After two treatments the smell was completely gone.

One more thing! It makes everything smell fresh and...well...like outdoors but better. Clean outdoors...maybe that describes it. It doesn't irritate your nostrils or offend you sense of smell or burn your eyes with any icky vapors.
