I noticed this one one of my year old Californian hens yesterday. She is limping slightly, but acting otherwise normal. My daughter says she thinks it has been there for a month, but I am not sure that is reliable. Bumble foot? I haven't ever seen it on the top of the foot before?
 
Yes, bumblefoot can occur on the top and bottom of the foot pads and toes. Can hou get a picture of the foot pad? It looks like there may also be a case of scaly leg mites. You can begin to soak the feet and legs daily in a dish pan or sink of warm Epsom salts or chlorhexidene and water for about 15 minutes. For the mites you can apply an oil, such as vaseline, mineral or castor oil and rub it into the scales. Once you get the scab on the top of the foot softened, you can try to remove it and try to squeeze out any core or solid material. If you cannot get it out, bumblefoot surgery where you cut the scab out with a scalpel blade, can be done. A dressing of a folded 2x2 gauze, plain antibiotic ointment, and strips of vet wrap can be applied. Change it every 2-3 days. Here is a good article about bumblefoot:
