I’m thinking this isn’t going to turn out okay

Where exactly is the screw hole in relation to the rest of her body?
It's very difficult to tell from the pictures.
If the screw is protruding from her crop then I would think it is possible she swallowed it.
If the screw is located elsewhere then it would seem possible that it hasn't passed through any internal organs. If this is the case then removal is an option.
Lots of advice on surgery. Making a small cut to remove an object from muscle is one thing, trying to remove an object that has pierced internal organs is altogether a different matter and should be left to a qualified vet.
Some recommendations for giving antibiotics and pain killers. According to the OP the hen is showing absolutely no signs of having an infection and from what little I can see from the pictures there is no sign of infection at the puncture site. The time to give antibiotics is when there is an infection.
Regarding the pain killers. For how long would you give painkillers for? Is the pain critical today and better in a couple of days time? If the hen lives then do you commit to a lifetime of giving pain relief?

Take some better photographs please. If necessary trim back some feathers from the wound site. State/show exactly where the screw exit hole is in relation to your hens anatomy.

Thing is, I can't see a screw like that passing through the gizzard and into the intestinal tract without causing so much damage that the hen dies. I'm inclined to think that the screw has penetrated the body from the outside. However, not actually being able to handle the hen and make a physical assessment makes giving any other advice than to cut off what is protruding and hope for the best irresponsible.
Bear in mind the length of the screw and the shape of the stomach and intestines. The stomach has a 360 degree bend in it and the intestines are close to coiled. I just can't see a screw like that passing through.:confused:

I agree. I have seen the results of birds eating stuff like this and it doesn't end well.
