Maybe so. But sometimes people over look things . Some times we see the worst when its really isn't that bad.
I find it hard to believe a screw is coming out of its guts and not affecting her . It doesn't look like it even rusted. You would think it would show signs of discoloring

Agree, trim it off.
Eating it may not have been how she acquired it in fact I doubt it.
I'm thinking it basically pierced her belly flesh and skin recovered the head.
The lump is likely an attempt on her body's behalf to push it out.
Don't fret, she'll be fine.

Can I just say...that is crazy!!! Animals survive all the time with weird stuff inside or through them

I'm going to sweep my coop to make sure there's no loose screws, I didn't think a chicken would eat one

Hate to state what’s been discussed previously. I agree that I don’t think it could have come from the inside. Is there any chance you can make a tiny incision and pull the whole thing out? Is the head of the screw right against her skin?

Yes, chickens do eat metal. Someone posted that she lost almost her whole flock from eating screws. . . some construction site is where she put her run and coop. It's here, you can do a search for it.
