I’m thinking this isn’t going to turn out okay

Hate to state what’s been discussed previously. I agree that I don’t think it could have come from the inside. Is there any chance you can make a tiny incision and pull the whole thing out? Is the head of the screw right against her skin?
You and I may try that, but I'd say most people would be a little too squeamish to do that on a chicken without anesthesia. . . What say you @BellaLunaTick ?

My son was fly fishing in the wind and got his own hook in his eyelid. He was 8 years old at the time. It didn't hurt him but Dad freaked and he started crying. I told Dad to shut up and back off! The hospital was near, they cut it off and pulled it through. Lucky it didn't puncture his eye. Dad had one in his ear once. Silly fishermen. . .

I would freak out as a dad too.
 
If you can feel the head close to the skin I would attempt to cut it out. The lump might be infection from it. I wonder if it might be a headless screw? Would explain how it got there from the outside. Whatever you do I wish you good luck. Its hard to take a $10.00 chicken to a $100.00 vet. I get that.
