I need help with the entry form for the Joint National

I can't make heads or tails of the entry form that's posted on the website for the joint national in Indianapolis in October. It might just be my computer, but the formatting on the document is really strange and I can't see all of the text on the entry forms. I need to enter ducks too, and the entry forms seem to only be for chickens. Can anyone help?

The first page is a little strange, but pages 9-12 where the entry forms are look ok on my computer. The first column is for the type of bird, which might be the part you can't see because it looks like you'd enter "D" there for ducks.
